Re-order prescriptions, preventative treatments, food and supplies online!

Please use the form below to submit your request.  By law, we can only fill medication requests for clients and animals that have a current veterinary-client-patient relationship.  This means that we must have seen the patient for an exam within the past 12 months, and have knowledge of the condition being treated.  This policy applies whether or not you submit your request with our hospital, or request refills from an independent online pharmacy.  Please note that by law, we can not accept returns on any medications, so please ensure that your order is correct. 

 

For additional requests for different pets, submit your first request and after seeing the "Thank You" page, just hit the "Back" button in your browser.  Your form info should show up again and you can just change your pet name and items needed.  When finished, hit "Submit" again and repeat as necessary.

Refill requests

Please be sure to include the name on the medication, the quantity requested (for example:  Trifexis 40-60 pounds, 6 months)

You will be contacted when your order is ready for pick up. If you would like your order shipped please list the shipping information below. You will be contacted for payment.

We will call you within 1 business day if we have any questions about your order. If you choose to pick up your order at the hospital, you may pay for your order at that time or pre-pay over the phone. If we are shipping the order, we can take your payment by phone. Please do not include your credit card number in this form.
